Immunometabolic, Vascular, and Epigenetic Determinants of Periapical Healing in Diabetes: A Narrative Review.
Uncontrolled high blood sugar impairs periapical healing through multiple interconnected mechanisms including microbial imbalance, blood vessel dysfunction, immune dysregulation, abnormal bone remodelling, and epigenetic changes in stem cells. Conventional root canal treatment alone may be insufficient in diabetic patients, suggesting adjunctive therapies targeting these pathways warrant investigation.
